Why Use Forms and Surveys?
Forms provide the web designer a perfect way to interact with those on the other side of the Internet.  Instead of making a web page that is merely a sounding board of the designer, the forms allow communication between the site's visitors to the designer.

Forms are usually submitted to an email address, but they can also be submitted to a database file or another web page.

Steps for Creating Forms
1.  Brainstorm the information you are looking for.

2.  Determine the type of form that would be useful and the fields you want to use.

3.  Create prompts, fields with field names, field properties, and where you want the information submitted to.  Use form wizard.

4.  Customize properties for the form.

5.  Publish the form online.

6.  Announce the publication of the form to the class or other audience members.
